I have read the 2011 tournament regulations, terms of play including the liability waiver, Conditions of Competition and Local Rules and will abide by them. I understand the eligibility requirements listed in this notice. I certify that I qualify as an amateur according to the Rules of Amateur Status of the RCGA, and I agree that this entry is subject to the approval of the GTAGA championship committee. In consideration for GTAGA services in conducting the regional qualifier/championship, I hereby grant and assign to the GTAGA without limitation, my individual television, radio, print advertising and all other related media rights with respect to my participation in the tournament.
